Have you ever attended the clinic and found that some of your details are out of date…. and then had to spend time updating them at reception before being able to sit down to wait for your doctor?
We have recently introduced a form which will be sent to your smart device as soon as you make an appointment -either online, over the phone or in person.
You will be able to complete the form online and return it securely to the clinic, where a member of staff will check the entry and update your profile.
The form will only be sent to you if you have not completed one in the last 6 months.
If you do not have a smart device, you can still update your details by using the self-check in devices located at each end of the reception area or by speaking with our reception staff.
We hope that this change will improve the accuracy of patient details we hold in clinic and patient through put.
